So you had a wasp waist and added a bustle. And you wore bloomers, stockings, possibly a horsehair and whalebone stiff underskirt foundation and a slip over that. You wore a camisole under the corset.

Ummmm… she looks like she has had ribs removed. Actresses sometimes have 2 ribs removed to create smaller waist.
There’s also a possibility she’s had surgery to remove a section of her intestinal tract. That has a drastic impact on nutrition deficiencies, metabolism and inability to gain weight regardless of food consumption. If so, there are a lot of risky health complications.
